PARENT GUIDE FOR PHYSICAL EXAMINATION AND IMMUNIZATIONS REQUIRED BY SCHOOL CODE OF ILLINOIS

GRUNDY COUNTY HEALTH DEPARTMENT IMMUNIZATION CLINIC - Immunizations for DPT, Polio, MMR, HIB, and Hepatitis B prevent these serious diseases and their crippling effects. The Health Department provides immunizations for the residents of the county every Wednesday . Call 941-3419 to make an appointment. Immunizations will be administered according to the recommendations of the Illinois Department of Public Health.

Failure to comply with the following requirements by the first day of school will result in your child not being able to attend school, unless exception is granted.

PRESCHOOL:
Effective August 1995, Lead Poisoning Prevention Act requires that any children registering for kindergarten or preschool program should show documentation that lead poisoning has been addressed by the health care provider. This is noted on the physical examination form.

I. Physical Examination by a licensed physician

II. Immunizations:

A. DIPHTHERIA, TETANUS AND PERTUSSIS (DPT)

B. POLIO MYELITIS (Oral Vaccine - TOPV)

C. MEASLES (Red or 7 Day), RUBELLA (German or 3 Day), MUMPS (MMR)

D. HEPATITIS B (series of three injections)

E. HIB

F. VARICELLA (Chicken pox)

ALL immunization dates must be posted on the Health Examination Form and signed by the health care provider certifying the immunizations were given.


FOR THOSE MOVING IN FROM OUT OF STATE:
A new Illinois physical exam is required. Forms are available at the office. There is a 30 day time limit to have these completed. A copy of your child’s immunizations will be needed at the time of the physical. If there are new immunizations that might be required, they will be started then.
